I have two maps that show this. From the end of Segment 2, where the CT intersects with Hwy 126, drive north on Jefferson County Rd 126 to Buffalo Creek. Then follow the North Fork of the South Platte River downstream on County Road 96 (it goes northeast for a few miles, then southeast, following the river the whole way.) This will take you to the North Fork's confluence with the South Fork and the trailhead. I'm seeing a Forest Road 538, but it is not clear if this is a reliable route. The folks at REI should also be able to help you out.
